Making Appointments
How to make/confirm your appointment:
- Decide your surgery date
- Email to make the appointment (below)
- Fill out and return pre-op papers
- Pay deposit and send your paperwork
- Schedule your transportation shuttle (if needed)
Making Appointments:
it is preferable to email the office to make your appointment, and call the doctor for medical consultations.
To make your appointment, please email us your preferred date / time, we will verify the availability and send you the paperwork for the appointment process. You may also fill out your papers in advance.
If you have medical records (not mandatory but helpful) you can send them directly to us by Email or Fax 1-281-520-3074 (we need the ‘procedure description’ and the pathology report if available). After the doctor reviews them we will give his report by email.
Medical/ Surgical consultations: If you would like to discuss your surgery or any medical issues with the doctor - you can call Dr. Levi at 011-52-899-871-1586 - or - 011-52-899-132-2614 - daily (including weekends) 8am-8pm central time (second number also works with Skype). If you make an appointment at this time please do not forget to email with your date to get your paperwork started.
Pre-Op instructions: If we have your correct information, you will receive your pre-op instructions approximately 5-7 days prior to surgery. Please email us ASAP if you have not received your instructions a few days before your appointment. If we do not hear from you the week before your reversal date, we may assume you are not coming.
Calling for Medical consultation / Emailing the office
Our physicians speak fluent English / Spanish. If you call for a surgical/medical consult (optional) you will not need a translator. A pre-op evaluation form will be sent upon scheduling or in advance if requested - the evaluation will allow the doctor to give you an estimated cost for your surgery. All patients must fill out the evaluation form.
Important! Even if you give your contact information during your call, you must email the office with your contact information and request your deposit invoice and pre-op papers. If your information changes, please notify us immediately. It is the patient's responsibility to maintain contact with our office. If we are unable to contact you, the appointment may be cancelled without notice.
You appointment is not complete until we have received your contact information by email and your deposit is made. See info page below. If you schedule and do not make a deposit, you may be charged a missed appointment fee to reschedule.

CANCELLATIONS:
2 very important details.
We respectfully ask that you do not make your appointment if you are unsure about the date, your funds, child care, time off work and various other issues. This is unfair to other patients and very inconvenient to the hospital staff and other doctors with patients who willingly arrange their schedules around international travelers. We understand that 'unexpected' problems can arise, but please contact us immediately for assistance if that happens. Please remember not to make unchangeable travel plans until your appointment has been confirmed. We are not able to schedule surgery 'around' your travel plans so be sure to make your appointment first, travel plans second.
Because we rely mostly on email communication, it is the patient's responsibility to make sure we have your contact information. If we cannot find you, we cannot work with you - so keeping in touch with the office is necessary.
CANCELLATIONS:
- To cancel your appointment for any reason, contact us via email in advance. 30 days notice for cancellations to obtain a partial refund of your deposit.
- If you miss your appointment without any notice* you will be charged a rescheduling fee/missed appointment fee. See the deposit page for complete guidelines.
- Rescheduling your appointment is fine, but please email us with your changes at least 2 weeks in advance.
- A new date should be chosen at that time and surgery must take place within 60 days of original date for deposit to apply.
- You may schedule 2 appointments on one deposit (initial appt + 1 reschedule). All subsequent appointments require a rescheduling fee of up to 50% of the original deposit. A full deposit may be required depending on the circumstances or the number of times an appointment has been missed.
- There is a considerable amount of time and paperwork that goes into scheduling your surgery, so patients who repeatedly schedule appointments but do not follow through with confirming will be charged a rescheduling fee in addition to the deposit for every missed appointment. Patients who habitually make appointments and do not show up for surgery may be charged a missed appointment/ rescheduling fee in addition to the deposit before being allowed to reschedule. If you are not sure you can pay the deposit and confirm the appointment - please wait to schedule the appointment when you are sure you can follow through.
- All third appointments, certain missed appointments and appointments that are cancelled for non payment of the deposit will be charged a fee to reschedule. We regret that this is an issue, but please remember if our surgical team and hospital staff spend their day preparing for your surgery and when you do not show up - a lot of people have worked on your behalf when they could have put their time to to use with other patients. The rescheduling fees pay for the OR scheduled time and administration fees that we have already paid on your behalf in order to schedule you for surgery.
